By completing this survey, you acknowledge that where you have given your consent MAS may contact you regarding your response to thank you for your participation, and/or in an effort to improve the products or services provided to you as a MAS Member. 

MAS Prize Draw Terms and Conditions 

By entering the MAS Net Promoter Survey (NPS) prize draw (the Prize Draw), you agree to the following terms and conditions: 

  1. The promoter of the Prize Draw is Medical Assurance Society New Zealand Limited, 10 Waterloo Quay, PO Box 957, Wellington 6140. 
  2. The Prize Draw is open to persons who have completed an NPS from MAS and conducted by Perceptive Group during the periods outlined in point 3 of these criteria. 
  3. The prize draw periods for completed NPS’s are 1 Jan to 31 March, 1 April to 30 June, 1 July to 30 September, and 1 October to 31 December in each year, until further notice. 
  4. A person receives one entry into the prize draw for each NPS they complete in each prize draw period. 
  5. The Prize Draw will be drawn at the closure of each prize draw period by a MAS representative. The winner will be 1 randomly selected person who meets the criteria set out in these terms. 
  6. The winner will receive a $50 Prezzee e-gift card and will be notified either by email or phone as provided in the Prize Draw form. 
  7. If the winner does not claim the Prize within 30 days, the Prize will be redrawn in accordance with these terms. 
  8. The Prize is not transferable. MAS reserves the right to change the date the prize is drawn, cancel the Prize Draw or exchange the Prize for a similar prize or equivalent value without notice at any time. The Prize cannot be exchanged for cash or other goods. MAS’s decision is final and no correspondence will be entered into. 
  9. Employees of MAS and its related companies (and their immediate families) are not eligible to enter this Prize Draw. 
  10. By entering the Prize Draw you agree that MAS may use your name and photograph for publicity reasons. 
  11. If a winner does not comply with these terms, MAS reserves the right to withdraw the Prize from that winner. 
  12. Variation: MAS reserves the right to vary any of these terms and conditions at any time without providing notice.
