Medical Assurance Society New Zealand Limited (MAS), PO Box 957, Wellington 6140, is collecting and will hold this information. MAS may use this information to send you marketing and/or promotional material where you have agreed to us doing so. You can access and correct this information under the Privacy Act 2020. 

By entering the MAS onMAS Survey Prize Draw (the Prize Draw) you agree to the following terms and conditions: 

  1. The Promoter of the Prize Draw is Medical Assurance Society New Zealand Limited, 10 Waterloo Quay, PO Box 957, Wellington 6140 
  2. The Prize Draw is open to MAS Members who have completed the onMAS Survey 2025 by 30 September 2025. 
  3. The prize will be drawn by a MAS representative on 1 October 2025.
  4. A person receives 1 entry into the prize draw by completing the survey and contact details section on the survey. 
  5. The winner will be 1 randomly selected person who meets the criteria set out in these terms. 
  6. The winner will receive a $500 contribution to a MAS Investment Funds account in their name (Prize) and will be notified either by email or phone as provided in the Prize Draw form. 
  7. If the winner is not already a MAS Investment Funds member, they will need to open an account in order to receive the Prize. The winner can do this by reading the MAS Investment Funds Product Disclosure Statement and completing an online application form at Medical Assurance Society - Investment Funds and meeting all customer due diligence requirements.  
  8. The $500 fund contribution will be made by MAS to a MAS Investment Funds account in the winner’s name. 
  9. No minimum fund balance the MAS Investment Funds is required to be eligible for the Prize. 
  10. If a winner does not claim the Prize within 30 days, the Prize will be redrawn in accordance with these terms.  
  11. The Prize is not transferable, MAS reserves the right to change the prize drawn, cancel the Prize Draw or exchange the Prize for a similar prize of equivalent value without notice at any time. The Prize cannot be exchanged for cash or other goods. MAS's decision is final and no correspondence will be entered into. 
  12. Employees of MAS and its related companies (and their immediate families) are not eligible to enter this Prize Draw. 
  13. By entering the Prize Draw you agree that MAS may use your name and photograph for publicity purposes.  
  14. Variation: MAS reserves the right to vary any of these terms and conditions at any time without providing notice.
